I hereby release, waive, and discharge Abrie du Plooy Tennis Academy, its owners, directors, coaches, staff, partners, facilities, and affiliates from any and all claims, demands, actions, or causes of action arising from participation in academy activities, including those resulting from ordinary negligence.

Medical Authorization: *

In the event of injury or medical emergency, I authorize Abrie du Plooy Tennis Academy staff to obtain emergency medical treatment if necessary. I accept responsibility for any medical expenses incurred.

Photo & Media Release: *

I grant permission for photographs and video recordings taken during academy activities to be used for promotional and marketing purposes, including website and social media use, without compensation.
